About the Documents
Name or type of each document being signed
Complete document package available
Approximate number of documents
Number of documents requiring notarization
Number of notarized signatures required
Type of notarization requested, if provided by the document or attorney
Any signing instructions provided by your attorney, title company, document preparer, or other requesting party
Whether witnesses are required
Number of witnesses required
Whether qualified witnesses have already been arranged
Any special signing or return instructions
About the Signer(s)
Full name of each person who will be signing
Number of signers
Valid, acceptable identification for each signer
Confirmation that each signer will be physically present for the appointment
Confirmation that each signer is willing to sign
Information regarding any mobility, communication, vision, hearing, or other accommodation that may affect the signing process
If Someone Is Signing Under a Power of Attorney
Please tell us before the appointment if anyone will be signing as an Attorney-in-Fact/Agent under a Power of Attorney.

The Power of Attorney document
Signing instructions, if provided
The name of the Principal
The name of the Agent/Attorney-in-Fact
The document(s) the Agent will be signing
Hospital, Hospice, Nursing Facility or Assisted Living Appointments
Please let us know in advance if the signing will take place in a healthcare or senior-care facility.
Be prepared to provide:
Facility name
Facility address
Room number, if available
Signer's current location within the facility
Facility access or visitor instructions
Whether witnesses have been arranged
Whether the signer can communicate directly with the notary
Any known circumstances that could affect the signing appointment
Important — Please Do Not Sign in Advance
Unless the particular notarization permits a previously made signature and the notary determines all legal requirements can be satisfied, please wait to sign until instructed by the notary. Documents requiring an oath or affirmation must be handled appropriately during the notarization.
